Questionnaires

• Please complete the questionnaires emailed to you.

• Depending on the assessment you will receive an ABAS, SRS and / or BRIEF questionnaires

School & Healthcare Reports

• Send school reports and healthcare reports to: reception@mindevolutioncentre.com or let us know if you’re dropping physical copies in prior to the assessment date

• Please medicate yourself / your child as prescribed

• If you / your child wears reading glasses, please bring these
